Medical Summary DVM Intake Exam 6/21/17 Estimated age: ~12yrs, male Microchip noted on Intake? No History : Stray Subjective: Observed Behavior – QAR, recumbent – able to move from lateral to sternal on own – allows all handling. Eating well Objective T = 100.3F P = 124 R = 24 BCS 3/9 EENT: Mature cataract OS, corneal scarring with neovascularization OD, Blind, ears clean, no nasal discharge noted Oral Exam: Severe dental disease – signicant tartar build up, moderate gingivitis and gingival recession PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpate U/G: Male MSI: Non-ambulatory, appears too weak to stand/walk, no palpable fractures or obvious injuries; unkempt, matted dirty coat, no masses noted CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric, underweight, severe dental disease, mature cataracts, blind, recumbent Plan: Place IVC with fluid rate of 10ml/hr, CBC/Chem Prognosis: Fair if responds to supportive care, poor long term prognosis SURGERY: Permanent waiver due to age and medical condition Addendum: CBC/Chem results Mild leukocytosis (18.78K/uL) characterized by a mild neutrophilia (14.66K/uL) with band neutrophils and a mild monocytosis (1.84K/uL); mild non-regenerative anemia (31.9%) with mild thrombocytosis (524K/uL) Severe azotemia (BUN >130U/L, creat too high to read) with marked increase in phosphorus (>16.1mg/dL); mild increased total protein (8.4g/dL), globulins (5.5g/dL) and amylase (1995U/L) Assessment: Inflammatory leukogram, renal disease, dental disease — patient appears brighter since intake, able to remain in sitting position, continues to have good appetite Plan; Continue with IVF therapy and monitoring in medical throughout day.
