Medical Summary DVM Intake Exam Estimated age: 5yrs Microchip noted on Intake? no History : surrendered by o because afraid of dog Subjective: Observed Behavior – lunged at camera when attempted to take ID foto, stiff posture, shaking hind legs Objective VERY LIMITED EXAM d/t temperment EENT: Eyes clear, ears clean, no nasal discharge noted, muzzle on H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, approx 8cm soft red/pink mass on L hind area, on caudal upper L: thigh- only permitted quick palpation CNS: mentation appropriate – no signs of neurologic abnormalities Assessment MI mass: r/o benign vs. malignant Plan ok for neuter once sedation- complete full exam and further assess mass on L caudal thigh Prognosis: open depending on underlying cause of mass SURGERY: Okay for surgery
