DVM Intake Exam; Estimated age:11-13; Microchip noted on Intake?n; History :stray. hematuria reported; Subjective: very large mass in right axillary area, possible lipoma (soft but more flucuant than normal). additional smaller lipomas present. black pendulous mass in right perianal area; Observed Behavior -shaking, cooperative; Objective ; T =; P =60; R =wnl; BCS 7/9; EENT: Eyes clear, au hyperpigmented, hyperkeratotic; Oral Exam:severe dental disease; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: no SS seen; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: hematuria (rule out stones vs uti), otitis, numerous masses; Prognosis:fair; Plan:abdominal rads, clavamox 125mng bid x 10d, mometamax x 1 week. recc FNA of masses. ohe, dental in future; rads: no stones seen, bladder not visualized; SURGERY: Permanent waiver due to age
