Medical Summary 05/24/17 DVM Intake Exam Estimated age: 4 yrs Microchip noted on Intake? No. History : Brought in by owner, no medical concerns noted. Subjective: Observed Behavior – Shy, very tense, allows limted handling while wrapped in a towel. Bites when face is examined. Objective: BARH. mm=pink, moist, CRT<2 s. HR=120, RR=pant. BCS=4/9. ORAL: Unable to fully examine – no obvious calculus or oral lesions. EENT: Eyes clear, no nasal discharge noted. Unable to examine ears – patient too mobile. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. No coughing. ABD: Non painful, no masses/organomegaly palpated. U/G: Female, no obvious spay scar although fur on abdomen is matted so difficult to visualize. MS: Ambulatory x 4, no lameness. INTEG: Matted haircoat face, ears, body and limbs. Unable to assess skin condition. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Apparently healthy except for matting. P: Schedule for sedation, mat shave. OK for OVH surgery. Excellent prognosis.
