Medical Summary 03/03/16 15:52 initial exam S/O: condition: severely matted haircoat with urine and feces incorporated in the matted fur, dehydrated (8-10%) heart: 4/6 left sided heart murmur, HR 180 (tachycardic) respiratory: lungs clear bilaterally, no nasal discharge or CIRDC signs musculoskeletal: BCS 1/9, severe diffuse muscle wasting, non-ambulatory on exam (weakly ambulatory on presentation), no wounds or injuries found oral cavity: mm’s pale pink and tacky, mild dental tartar build-up eyes: clear OU, dried mucoid ocular discharge OU ears: matted pinnae, ear canals clean AU neuro: no neuro deficits noted, full exam not performed abdomen: soft and non-painful, no masses palpated rectal exam: not performed urogenital: male intact, 2 scrotal testes A: severely matted emaciated body condition (BCS 1/9) dehydrated P: BG: 179 Placed IVC and started LRS + 20 mEq KCl/L @ 10 ml/hr (less than 2 X maint due to heart murmur) fed small amount of a/d – very strong appetite! offered water – very thirsty! start modified refeeding plan: feed 1 tablespoon of wet food TID for 2 days, then 2 tablespoons wet food TID for 2 days, then 3 tablespoons of wet food TID x 7 days suspect cruelty – waiting on follow up needs grooming once stable start famotidine 1/4 tab BID
