Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective : P = WNL R = WNL BCS 4/9 EENT: KCS, crusted mucoid ocular discharge OU, ears clean, no nasal discharge noted Oral Exam: Severe dental disease, pink mm PLN: Mildly enlarged submandibular lymphnodes H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, thin, unkempt thin hair coat, abrasions at inguinal region CNS: mentation appropriate – no signs of neurologic abnormalities — trembling throughout exam Assessment: Geriatric, dental disease, KCS, blind, underweight+/- PU/PD, +/-neurologic Plan: CBC/Chem/T4, UA scheduled. Continue to monitor Prognosis: Fair/open pending diagnostics
