Allowed all handling but slightly head shy.  Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective : P = WNL R = WNL BCS 4/9 EENT: KCS, crusted mucoid ocular discharge OU, ears clean, no nasal discharge noted Oral Exam: Severe dental disease, pink mm PLN: Mildly enlarged submandibular lymphnodes H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, thin, unkempt thin hair coat, abrasions at inguinal region CNS: mentation appropriate – no signs of neurologic abnormalities — trembling throughout exam Assessment: Geriatric, dental disease, KCS, blind, underweight+/- PU/PD, +/-neurologic Plan: CBC/Chem/T4, UA scheduled. Continue to monitor  Prognosis: Fair/open pending diagnostics

RE EXAM: Assessment:  Geriatric; Dental disease; KCS; Blind; Cachexia; Overgrown nails; Azotemia r/o kidney disease; Elevated liver enzymes;  Plan:  CTM while at BACC; CBC-mild anemia 35.2 (37.3-61.7), mild monocytosis 1.34 (0.16-1.12), mild neutrophilia 12.64 (2.95-11.64); UA pending collection; Chem-ALP 661 (23-212), elevated ALT 375 (10-125), azotemia BUN 69 (7-27), creatinine 2.1 (2.5-4.5),; hyperglobulinemia 4.9 (2.5-4.5); Clean eyes and trim nails today; STT; -OD-5; -OS-3; Start optimmune OU BID x14d until 12/6; Start erythromycin OU BID x14d until 12/6; Start LRS 20ml/kg SQ BID; Rec hospitalization and internal medicine workup; Recheck daily
