I came in the shelter as a OWNER SUR on 12/22/2016 from NY 11377, owner surrender reason stated was NO TIME.

12/23/16 S/O: BAR, friendly, docile geriatric dog, food eaten EENT: moderate tartar and gingivitis, nuclear sclerosis OU, mild mucopurulent d/c from OU, otherwise eyes open and comfortable, pink gums, waxy ears M/S: BCS 3/5, amb x4, but significant limp from likely L FL. Bowed legs in FLs bilaterally, severe carpus valgus L FL, no crepitus or pain elicited when palpated, good ROM. R HL- MPL 2/4, good ROM in hips bilaterally. approx 3cm soft, non painful SQ mass on ventral thorax SKIN: alopecia along medial FL’s and chest, pigmentation of the skin and very mild lichenification but no epidermal collarettes oily fur between shoulder blades- r/o from Activyl administration A: deformities in forelimbs with L FL limp- r/o secondary to deformities as apparently non painful, other MPL 2/4 R HL OU: bacterial eye infection r/o secondary to KCS, other nuclear sclerosis OU moderate dental and periodontal dz mass ventral thorax: r/o benign vs. malignant skin: r/o underlying metabolic/endocrine disease, other P: OU erythromycin OU BID x7days, then consider STT will pursue rimadyl short trial to determine if helpful with L FL limp in case of occult pain: 50mg/ml: 0.1ml SQ SID x3 days rec cbc/chem/t4/ua/fecal OPG as general health screen, consider ACTH stim to r/o Cushing’s consider FL radiographs, joint supplements mass on ventral thorax: rec aspirate/cytology to determine underlying cause long term rec dental px clean ears prognosis: fair- p has underlying issues, but can be kept comfortable. 12/22/16 13:24 basic information Kato is a 12 year old gray and white shitzu. He is an altered male. Owner bought him from a pet store and has had him since a puppy. Owner stated that he has difficulty walking on his back legs. He has not seen a vet for his health issues. Socialization around strangers he is friendly and outgoing and likes to be pet on head. He has not lived or spent time with children younger than 13 years old. He plays gentle with adults and play behavior is unkown with children. He has spent time with a younger maltese and does well with them. There is no bite history of a person or animal. Behavior There are no behavior issues owner is concerned about. He isnt bothered with storms or fireworks.He isnt bothered with touching his food bowl, taking a toy out his mouth. He enjoys bathing, brushing and being picked up. He isnt bothered with trimming his nails. For the new family to know Owner described him as friendly and mellow. Owner stated that his favorite thing about kato is that he is the perfect dog and has always been a great companion. When owner is home he stays in his favorite spot. He doesnt play with toys or plays any games. His activity level is low. He eats rachel ray dry dog foos with wet pedigree mixed. His favorite treats are biscuits. He is very house trained and never has accidents in the house. he didnt have access to a yard and was kept mostly indoors. He sits sometimes when he wants to listen. He has never been crate trained and is well behaved in the house alone. He likes slow short walks onleash. He doesnt pull on leash and owner has never had him off leash. Behavior upon intake Kato was receptive to all handling including collaring, scanning, intake photo, and picking up.

A volunteer writes: “Kato is the perfect dog and has always been a great companion..” The words of his long time master who relinquished him for no time after 12 years of service. …..For my own peace of mind, I dearly hope that there is more to his relinquishment than meets the eye… Kato is an old pooch who came in with eye discharge, yeasty ears and unbalanced hips, making his gait awkward and difficult. He does do his business outside, though, enjoys a great weight and came in well groomed. He has the greatest appetite for dry and wet food as well as for treats and rotisserie chicken… Kato’s former family had only wonderful things to say about him. Of note is that he never saw a Veterinarian. In our hands, he displays no handling issues, seems fine with other dogs met and is a GREAT lap dog who loves to be petted. Kato is really cute, resembling a fluffy little stuffed toy , quite friendly and sociable . He is an elderly who comes understandably with age related issues and maybe some others. He is a sweet and adorable little guy with still some good time ahead of him in the care of a loving and savvy new owner. Kato is waiting for you at the Manhattan Care Center.
