Subjective: BARH, tail tucked, whale eyed, allows full exam; Objective; P = wnl; R = wnl; BCS = 4/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: mild to moderate dental tartar; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: MN; MSI: Ambulatory x 4, skin free of parasites, no masses noted, several circular areas of alopecia on front paws – each <1cm with central scabbing; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: Externally normal; Assessment: 1.Skin lesions – ro pressure sores vs allergy vs other; 2. Underweight – ro poor diet vs other; Prognosis: Good; Plan: Monitor weight and skin lesions while at BACC
