Medical Behavior: Blue,  Age: 4 years,  Sex: Neutered Male,  Weight: 47 lbs

DVM Intake Exam; Estimated age: Approximately 3-5 years based on dental tartar; Microchip noted on Intake? Positive scan; History : Adopted in March 2018 from ACC. Brought in by police. Lost 10 pounds since adoption 1.5 months ago. Subjective: BARH, tail tucked, whale eyed, allows full exam; Objective; P = wnl; R = wnl; BCS = 4/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: mild to moderate dental tartar; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: MN; MSI: Ambulatory x 4, skin free of parasites, no masses noted, several circular areas of alopecia on front paws – each <1cm with central scabbing; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: Externally normal; Assessment: 1.Skin lesions – ro pressure sores vs allergy vs other; 2. Underweight – ro poor diet vs other; Prognosis: Good; Plan: Monitor weight and skin lesions while at BACC

No; History : Stray; Subjective: BARH; Observed Behavior – Active, exploring room, pulls hard on leash, allowed all handling for exam but became slightly tense, soft muzzle placed for vaccines/blood draw – Jupiter allowed all procedures but remained very stiff with tail tucked throughout; Objective ; P = WNL; R = WNL; BCS 5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: Mild dental staining, pink mm; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Male; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat – small healed wound/scar above right eye; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: Apparently healthy; Prognosis: Excellent; Plan: Continue to monitor while at BACC; SURGERY: Okay for surgery
