Medical Summary DVM Intake Exam Estimated age: 1-2 years History : returned by previous adopter due to owners allergies Subjective: QAR Observed Behavior – resists oral exam but overall a very sweet girl and easy to examine otherwise Objective T =not taken P = WNL R =WNL BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, moist; CRT <2 sec; mild tartar, crowding of premolars; underbite PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: spayed MSI: Ambulatory x 4; grade II patellar luxation on L side with mild crepitus and no discomfort or associated lameness skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment 1. patellar luxation Plan rec’d weight management long term to prevent progression of patellar luxation; if it progresses in the future or becomes painful or causes lameness, then surgery for patellar stabilization should be pursued-this overall has an excellent outcome if needed Prognosis: good to excellent SURGERY: spayed
