DVM Intake Exam; Estimated age: Microchip noted on Intake? ; Objective ; T =; P =; R =; BCS; EENT: both eyes have meibomian gland growth on lateral aspects, ears clean, no nasal or ocular discharge noted; Oral Exam: mod dental tartar; PLN: No enlargements noted; H/L: NSR, heart murmur 2/6, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: intact male; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: heart murmur, meibonian gland masses; Prognosis:fair; Plan: recc ophtho consult if eyes become irritated, recc echo and neuter in future; SURGERY: permanent waiver due to heart murmur
