Medical Summary 05/03/17 DVM Intake Exam Estimated age: 9 yrs. Microchip noted on Intake? No. History : Found as a stray, noted to be matted. Subjective: Observed Behavior – Tense, shy, allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: QARH. mm=pink, moist, CRT<2 s. P = 116 R = 20 BCS=5/9. ORAL: Mod. calculus, gingivitis, gingival recession. EENT: Nuclear sclerosis OU. No oculonasal discharge. AU – erythema, tan discharge, pruritis. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/organomegaly palpated. U/G: Intact male, both testicles palpable. MS: Ambulatory x 4, no lamenss. INTEG: Matted over limbs, ears, cadual body, rear end. Feces stuck in matted perineal fur. Scrotal skin erythematous, moist, sensitive. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Age est 9 yrs old. Dental disease. Otitis externa AU. Scrotal dermatitis/pyoderma. Matted. P: Rx Clavamox 125 mg 1 PO BID x 14 days. Tomorrow: Sedate for grooming, ear cleaning and apply Osurnia AU. Good prognosis. Waiver from neuter surgery due to age.
