DVM intake; S/O: QAR 5-7% dehydrated; intermittently holding head as if weak/neurologic; drooling, blood tinged (normal oral cavity); no interest in eating canned or dry food; very interested in drinking water; very full urinary bladder – very concentrated; making moaning sounds throughout exam; 1/9 BCS; T: 101.6F; EENT: clear OU/AU, moderate tartar, no nasal d/c; PLN: wnl; H/L: wnl; GI: doughy on palpation, prominent urinary bladder, no masses palpated; UG: MI; MSI: weakly amb x 4, prefers to rest in sternal recumbency. IVC placed in front limb. right hind limb, 2nd digit nail broken; Neuro: alert and appropriate; Assessment: Adult MI Large breed dog; 1. no appetite; 2. drooling/nausea; 3. emaciated; 4. dehydrated; 5. dull mentation/weak; 6. high wbc count; 7. proteinuria (3 plus); 8. elevated ALP (355) r/o secondary to emaciation vs other; 9. hypoalbuminemia, anemia r/o secondary o emaciation vs underlying condition; 10. non-specific discomfort/pain; Plan: Continue on IVF at 75ml/hr; baytril 210mg IV – given; cerenia 21mg IV – given; mirtazapine 1/4 of a 15mg – given; recheck tmrw
