I came in the shelter as a OWNER SUR on 12/27/2016 from NY 11213, owner surrender reason stated was NYCHA BAN.

01/06/2017 Exam Type RE-EXAM – Medical Rating is 3 C – MAJOR CONDITIONS , Behavior Rating is EXPERIENCE, Weight 62.4 LBS.
01/06/17 Monitor for vomiting/CIRDC S/O: QARH. Nervous, allowed handling. Eating well. 2-3 episodes of regurgitated food yesterday, 1 episode this morning (cleaned prior to rounds) EENT: Mild conjunctivitis and moderate scleral injection OU, mild to moderate serous nasal discharge HL: No murmurs/arrhythmias, lungs auscult clear; unable to ellicit cough, sneezing MS: Ambulatory x 4, no apparent lameness INTEG: WNL NEURO: After pulling on leash to leave room during exam, Jeter slowly fell over and took about 5 seconds to right himself, after brief epsiode was again mentally appropriate with no ataxia (similar episode reported yesterday during adoptions interaction) UG: Neutered A: CIRDC; hx of vomiting consistent with hacking associated with CIRDC; episodes of collapse/falling over not previously reported prior to yesterday – rule out syncope vs seizure disorder vs other P: Rec doxycycline 300mg po q24 x 10 days. No treatment for eyes at this time due to nervous behavior displayed during exam. Continue to monitor for epsiodes of collapse. If continues, rec full neurologic work up. Good prognosis at this time

12/27/16 19:49 Jeter is a two year old gray and white neutered American Pitbull mix. He was surrendered due to owner residing in a NYCHA facility. Jeter was given to owner by a friend and was with his current owner for about two months. Jeter has no known medical issues. Jeter has lived with two adults and two teenagers. He is described as relaxed and playful with the teenagers and is friendly towards strangers. Jeter will engage in exuberant play with the teenagers and adults. Jeter has spent time with younger children ages 2 and above and was said to be relaxed and playful towards the younger children as well. Jeter has been around all breed dog and owner stated he will bark at first then jump and try to play with them .He has not been around cats. Jeter has not bitten another animal or person. Jeter is walked twice a day and is used to leash walks. He will pull hard on leash according to owner. Jeter is not bothered when his food is touched while eating, or when a treat or toy is taken away. Jeter tolerates baths and being groomed. Jeter has a medium to high activity level and likes to play with balls and stuffed toys. Jeter eats Beneful dry food and is fed once a day, he loves bacon and cheese. Jeter knows the commands sit and turn around. Upon intake Jeter had a loose wiggly body and allowed all handling. Cousnelor was able to collar, scan and photograph.

KNOWN HISTORY: Altered male, Owner surrender Previously lived with: Adults (2) and teenagers (2) Behavior toward children: Relaxed, playful, and exuberant with the teenagers and toward younger children (exposed to children ages 2+) Behavior toward dogs: Jeter engages in play with the other dogs. Behavior toward cats: Unknown FG/RG: None reported Bite history: None reported Housetrained: Yes, Jeter is walked daily. Energy level/descriptors: Jeter displayed a medium to high energy level. Other notes: SAFER ASSESSMENT: 12/31/16 Look: 2. Dog’s eyes are averted. His body posture is tense and unfocused, his tail is down. He allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 2. Dog stands still and accepts the touch, his eyes are averted, his tail semi-tucked, body tense, mouth, ears back. Tag: 2. Dog is not fearful, but is unresponsive to the Assessor, and approaches the Assessor at the end of the game (may need coaxing with e treat.) He is focused on stimuli other than the Assessor. Squeeze 1: 1. Dog does not respond at all for three seconds. His eyes are averted and his ears are back. Squeeze 2: 1. Dog does not respond at all for three seconds. His eyes are averted and his ears are back. Toy: 1. Minimal interest in Toy, dog licks, then turns away. Summary: Jeter was a bit distant; he was not sociable, but did not display any concerning behavior. He can do well in a home where he is more comfortable and relaxed. PLAYGROUP: According to Jeter’s previous owner, Jeter has been around all breeds of dog and owner stated he will bark at first then jump and try to play with them. MEDICAL BEHAVIOR: 12/28/16 During his initial medical examination, Jeter was nervous and the medical team performed limited handling. RECOMMENDATIONS: Experience (suitable for an adopter with some previous dog experience) Note: Jeter has displayed no aggression in the care center, but he has generally been a bit aloof and distant, and has not sought comfort from his human caretakers. We therefore feel that an experienced adopter who will allow Jeter to warm up to his new family and new environment at his own pace will best set Jeter up to succeed.
