Scanned negative History : DOH-B Subjective: Stands in exam room – then when I make eye contact, he crouches and defecates, later urinates. Pt snaps when we bring out a muzzle – then barks loudly. Rope muzzle was used and pt alligator-rolled several times. Nylon muzzle applied over rope muzzle – pt stared, had very stiff body and growled throughout the exam. Objective BAR-H, BCS 5/9, MMs pink EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Not performed PLN: No enlargements noted H/L: NSR, NMA Lungs clear, eupnic ABD: Tense, unable to palpate U/G: Neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: APH male neutered dog Plan: Vaccinate Prognosis: Good 1088
