Medical Summary DVM Intake Exam Estimated age: 8 reported Microchip noted on Intake? n History : o reports three day history of mass on rear end Subjective: right perianal area very swollen, hard erythematous with large draining tract. Anal sac abscess likely but cannot rule out neoplasia Observed Behavior – calm, allows full exam Objective P = 80 R =wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: clean teeth PLN: No enlargements noted H/L: NSR, NMA, , Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, few matts on tail CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: wnl. right side on rectum inlammed but wnl for this condition Assessment: AS abscess, rule out neoplasia Plan: sedated flush of area and antibiotics/anti-inflammatories. if rapid resolution does not occur, biopsy needed. recc neuter asap Prognosis: good SURGERY: Okay for surgery

PROFILE :
Basic: Jeremiah is an 8 year old medium sized male dog. His owner has had him since 2009 and got him from a family member whose dog had puppies. He is being surrendered due to housing issues. He has not had any illnesses or injuries. He has not been to the vet.
Socialization: Around strangers Jeremiah is friendly, affectionate, and playful. He has lived with and spent time with children as young as 5. With them he is relaxed and playful. He plays exuberantly with both adults and children. He is relaxed and playful with small and large dogs and plays with them exuberantly. He has lived with a cat and was relaxed and playful with the cat. He does not have a bite history.
Behavior: Jeremiah will poop and pee inside the house, mostly because his owner has never taken him out of the house. He is nervous during storms and fireworks. He isnt bothered when someone touches his food, takes away a treat or toy, pushes/pulls him off furniture, holds or restrains him, disturbs him while sleeping, brushes him. He loves baths once you get him into the tub. He is friendly when someone unfamiliar approaches his family member and house. He will bark when the doorbell rings.
For New Family: Jeremiah is described as friendly, affectionate, playful and sometimes fearful. His activity level is high. He loves to run around the apartment for exercise and play with his toys. He likes balls and stuffed toys to play with. He follows his owner around the house and sleeps in his owner’s bed. He likes to eat dry food. He is not housebroken and has accidents at home on a daily basis. He has been an indoors only dog. When he is left alone he will bark and whine for a bit but otherwise is well behaved. He has never been off leash and likes to run around the house for exercise.
During intake Jeremiah was very sweet and affectionate. He allowed all handling from the counselor and was curious about his surroundings. He was active in the intake room and was walking around and up to the counselor for affection.
