Medical Summary DVM Intake Exam Estimated age: young adult, ~ 2 yr Microchip noted on Intake? scan negative History : found stray with another dog (A1119543), injured + appears to have bite wounds Subjective: BARH Observed Behavior – friendly, allowed PE Objective T = 103 P = wnl R = wnl BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: nsf PLN: swollen submandibular region LN, nosf H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact female, prominent mamma and vulva – r/o past litters MSI: Ambulatory x 4, appears to have old healing abrasions and small punctures around right elbow consistent with bite wounds, sensitive on palp.; the right side of the face has many superficial healing abrasions and the submandibular region is mildly swollen also consistent with bite wounds; the left side of the face and left front leg also have small scars; no parasites observed, otherwise healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment – multiple small healing/infected abrasions and punctures around right side of face/ear/muzzle + submandibular region – small puncture/healing wounds at right elbow, tender to touch – injuries are consistent with bite wounds. Plan – gave 375mg Clavamox PO + 2 ml rimadyl sq during PE – clavamox 375 mg PO BID x 10 days – rimadyl 50 mg PO BID x 5 days – clean + recheck wounds Prognosis: good SURGERY: Temporary waiver due to injury + DOH hold
