Medical Summary DVM Intake Exam Estimated age: 8 years per our previous estimates Microchip noted on Intake? Scanned positive History : The owner days she has noticed drops of blood in Princess’ urine the last few months but she could not afford to take her to the vet. Today, the owner noticed and overwhelmingly large amount of bloody urine and she decided it was best to surrender. Subjective: Alert, calm, walks around Observed Behavior – Relaxed, allows all handling Objective BAR-H, MMs pink and moist, BCS 6/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Severe tartar, gingivitis and gingival recession PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, spayed (spay scar palpable and spayed as per our previous records) – hematuria visible on floor as pt walked into the room. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment 1. Hx hematuria for several months, it has suddenly gotten much worse – R/O simple UTI vs. ascending infection vs. bladder stones Plan: 1. Clavamox 125 mg PO BID x10 days 2. Abd rads and CBC/chem to look for stones and confirm that kidney function wasn’t compromised. Sedation okay if needed. Prognosis: Good with appropriate treatment 1088
