Medical Summary scan positive –985112005312956 ~12-15yrs, spayed female S/O: QARH. Allowed all handling, attention seeking. BCS 3/9 EENT: Severe dental disease, significant gingival recession, severe tarar buildup, halitosis, nuclear sclerosis, no ocular or nasal discharge, ears wnl HL: Normal thoracic auscultation, no murmurs/arrhythmias, lungs clear ABD: Normal abdominal palpation INTEG: Severely matted and dirty coat especially at distal limbs, right hip and hind end MS: Ambulatory x 4, no apparent lameness UG: Spayed female, spay scar seen, small nipples A: Geriatric, underweight, severe dental disease, matted coat P: Rec clindamycin 3ml po q12 x 7 days. Rec dental cleaning with extractions with placement OK for sedated shave down. Continue to monitor while at BACC. Good short term prognosis
