Owner surrender, no medical history, BAR, euhydrated, ambulatory x 4, nervous and head-shy, but allowed all handling with moderate restraint BCS 6/9 EENT: Moderate mucopurulent discharge and conjunctivitis OD, ears clean, no nasal discharge noted Oral Exam: moderate/severe dental tartar and gingivitis PLN: No enlargements noted H/L: NSR, NMA, strong synchronous pulses, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male, two inguinal testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: bilateral cryptorchid, conjunctivitis OD, dental disease Plan: start Neo-Poly-Bac OU BID x 7 days, ok to be made available for adoption Prognosis: good SURGERY: Permanent waiver due to geriatric
