History :owner surrender; owner states that he had him for approximately 3 years and that jay jay has a history of inappropriately urinating in the house; Subjective:QAR; Observed Behavior -docile and friendly, allows handling; Evidence of Cruelty seen -in grown toe nail on digit III, LF Objective; BCS=4/9; EENT: Eyes clear, ears clean, no nasal discharge noted; Oral Exam:mm pk, sl tacky; CRT <2 sec; severe dental tartar; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G:male intact; prostate severely enlarged but smooth, soft and nonpainful; 2 scrotal testicles that palpate normally; MSI: Ambulatory x 4, skin free of parasites, no masses noted, mildly thing hair coat along caudal hindlimbs; severely in grown toe nail on digit III of LF; corresponding wound appears somewhat chronic with granulation tissue present; CNS: mentation appropriate – no signs of neurologic abnormalities; Rectal:enlarge prostate; feels smooth and uniform, soft and nonpainful; Assessment; History of inappropriate urination-r/o secondary to prostatomegaly vs other; Prostatomegaly-r/o BPH vs neoplasia; Dental Disease; In grown toe nail; Prognosis:fair to good; suspect BPH which should resolve with neuter; Plan: Convenia 0.7 ml SQ; Carprofen 50 mg/ml: 0.6 ml SQ; Monitor wound to ensure appropriate healing; CBC/chem, neuter if WNL; SURGERY: Okay for surgery

Look: 1. Dog’s eyes are averted, ears are back, tail is down, relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands.
Sensitivity: 1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.
Tag: 1. Dog follows at the end of the leash, body soft.

Summary: Jay Jay approached the assessor in a calm manner when in the assessment room. He allowed all handling and displayed no concerning behaviors. He was, at times, focused on the exit.

Summary (1): 10/3: When introduced off leash to dogs, Jay Jay follows and postures to mount female dogs. He approaches and displays brief playful behavior around males.

Summary: Upon intake, Jay Jay had a wiggly body and allowed handling.
MEDICAL BEHAVIOR:
Date of initial: 3-Oct-2017
Summary: During his initial medical exam, Jay Jay was social and allowed handling.
ENERGY LEVEL: We have no history on Jay Jay so we cannot be certain of his behavior in a home environment. In the care center, he displays a medium level of activity.

Potential challenges comments: House soiling: Jay Jay is reported to have frequent accidents in the house, and appears not to have been housetrained in the past. He will need guidance and consistency to learn to eliminate outside. We recommend crate training (the crate must be made positive and never used as a punishment), frequent walks, rewards for eliminating outside (treats, toys, games), consistent feeding schedule, and careful monitoring when inside. Accidents should never be punished as it can damage the human-dog relationship and is likely to make the problem worse.
