Intake condition GERIATRIC Intake Date 05/09/2017, From NY 11691, DueOut Date 05/12/2017, I came in with Group/Litter #K17-096631
Medical Behavior Evaluation GREEN
Medical Summary Estimated age: 14 yrs Microchip noted on Intake? No. Found as a stray, no medical concerns noted. Friendly and social, very relaxed and easy to handle. BARH. mm=pink, moist, CRT<2 s. HR=116, RR=pant. BCS=5/9. ORAL: Missing all incisors and several premolars. Mod. calculus and gingivitis. EENT: OU- central corneal pigmentation OU. No discharge or chemosis. No nasal discharge. Mild upper repiratory stertor (consistent with brachycephalic conformation). PLN: No enlargements noted. H/L: Lungs clear, no dyspnea. Intermittent gr 1/6 left sided murmur. Difficult to fully auscult due to upper repiratory noise. ABD: Non painful, no masses/organomegaly palpated. U/G: Intact male, both testicles palpable. MS: Ambulatory x 4, no lameness but appears stiff in hind limbs with decreased hip range of motion bilaterally, L worse than R. INTEG: Full, somewhat dirty haircoat. No skin lesions or masses noted. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Approx. 14 yr old Pug with dental disease, likely hip arthritis, and exposure keratitis. Possible heart murmur – most likely cause would be mitral valve insufficiency. P: Rabies, DA2PP, Bordetella vaccine. HWT and Microchip. Pyrantel 0.9 ml PO, Applied Activyl. Permanent waiver from neuter surgery due to age. Schedule for CBC/CHEM. Start NSAIDs for arthritis if lab results look good. Good prognosis with apporipriate management.
