Medical Summary DVM Intake Exam Estimated age: 1-2 yrs Microchip noted on Intake? none History : owner surrender Subjective: BAR Observed Behavior – growling, snapping; muzzled for exam; growled and snapped when trying to evaluate mouth with tongue depressor but okay for remainder of exam with muzzle; allowed petting and holding after exam (muzzled) Objective T =note taken P = WNL R = WNL BCS 6/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm/pk, moist; unable to examine any further due to temperament PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact; 2 normal descended testicles MSI: Ambulatory x 4; grade III/IV medially patellar luxation on R side with no associated lameness; skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 1. Grade III MPL R side Plan no treatment needed Prognosis: good with weight management for MPL SURGERY: Okay for surgery
