DVM Intake Exam: Estimated age: ~9yrs Microchip noted on Intake? No History : Owner surrender — reportedly got leash stuck around hind limb when the owner was at work all day. Subjective: QARH Observed Behavior – Very calm for exam and procedures. Evidence of Cruelty seen – No. Evidence of Trauma seen – Yes Objective: P = WNL R = WNL BCS 5/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: Moderate dental tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female MSI: Ambulatory x 3, significant swelling of LH limb from thigh to paw, bruising along medial thigh, dark discoloration ofleft paw with serosanguinous discharge between digits 2-3, skin free of parasites, possible femoral fracture palpated, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities. Assessment: Injured LHL – R/O strangulating lesion +/- femoral fracture Prognosis: Good with appropriate treatment. Plan: Radiographs pending. Buprenex 0.24ml SQ and rimadyl 0.2ml SQ. NH seeking placement SURGERY: Permanent waiver due to age and injury

Basic Information: JaJa is a small tan and white mixed breed. She was given to the owner by a friend and she has had her for 9 years. She is being surrendered because she cannot afford the vet care and has no time.

How is this dog around strangers? JaJa is noted to be very shy with strangers. She may bark but runs to hide and takes her a couple of days to get use to people.
How is this dog around children? JaJa has not lived with children.
How is this dog around other dogs? JaJa has not lived with dogs.
How is this dog around cats? JaJa has not lived with cats.
Resource guarding: JaJa will not let go of items in her mouth and will snap and growl when her food bowl is touched.
Bite history: JaJa has not bitten a person or another animal.
Housetrained: Partially

Other Notes: JaJa is wee wee pad trained but misses a couple times a week.
Has this dog ever had any medical issues? No
For a New Family to Know: JaJa is described as a diva that loves attention and playing with her squeaky toys. She was fed dry food only. She is well behaved when left alone. She likes slow walks with her owner.
