Medical Summary Estimated age: 2 years Microchip noted on Intake? neg. History : was found last night tied to a tree; at that time, he was noted to be in good health and have full use of all limbs; finder took him home last night; this morning when they came home from church, he was unable to use his hindlimbs. BAR, sweet boy, allows all handling BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, mild hypersalivation; mild tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact MSI: skin free of parasites, no masses noted, healthy hair coat; no obvious orthopedic injuries-see neuro notes CNS: mentation appropriate hind limb paralysis, non ambulatory; absent proprioception, reflexes and deep pain in hind limbs; extensor rigidity in forelimbs tense on palpation of TL junction spasmodic urination during spinal palpation; expresses bladder on gentle palpation of abdomen Rectal: anal tone present, poss decreased Assessment Acute paralysis Plan gave 0.4 ml dexamethasone 2 mg/ml SQ gabapentin (250 mg/5 ml)-0.2 ml PO TID express bladder q6h rec’d NH placement with immediate transfer to neurologist for MRI +/- surgery Prognosis:poor to guarded due to deep pain neg status SURGERY: temp waiver d/t paralysis
