Medical Behavior Evaluation BLUE
Medical Summary Estimated age: Reported to be 17 years old – exam is consistent with this Microchip noted on Intake? Scanned POSITIVE on exam.  No medical hx given.  Timid, shaking, tense. Allows most handling but seems tense. Restrained gently with a towel for vaccines and blood draw.  BCS 4/9, MMs pink EENT: Eyes cloudy – nuclear sclerosis or cataracts. Mild brown waxy debris AU. No nasal discharge noted. Oral Exam: Severe tartar accumulation, halitosis – missing some teeth, others are pointing in abnormal angles. H/L: Grade 3/6 systolic heart murmur, PMI left base. NSR. Lungs clear, eupnic ABD: Tucked up, tense. No masses palpated. U/G: Female, reported spayed. Small undeveloped nipples and vulva. Two small semifirm masses associated with the left and right caudal mammary glands (2nd to last set). MSI: Hind legs are trembling throughout exam. Decreased muscle mass in hind limbs. No crepitus noted in stifles, fair range of motion in both stifles and both hips. Two soft pedunculated masses hanging from the left front leg at the level of the mid-antebrachium. The masses are primarily pink but also have dark pigmented regions. They are soft and hang about 5 cm long. Long toenails, especially on front feet, especially digits 3nd 4 on the right front foot. Skin slightly erythematous especially at dorsal neck. CNS: Quiet, passive. Mentation appropriate – no signs of neurologic abnormalities Assessment 1. Severe dental disease 2. Soft tissue masses on left front leg 3. Small mammary nodules 4. Heart murmur 5. Trembling – R/O discomfort from standing, stress, muscle disease, other Plan 1. Recommend chest rads and possibly echocardiogram – cardiac meds may be indicated 2. If anesthetically safe, consider dental cleaning and extractions of many teeth – pt will likely be much happier once this source of pain is gone 3. Consider FNAs of mammary masses and pedunculated masses on LFL 4. CBC/chemistry Prognosis: Fair 1088

Jackie likes to rest in her kennel. Once out, though, she is surprisingly quite active. She is a great walker and does her business on the way(we are told that she is mostly house trained with some accidents). She explores but follows when called. She climbs stairs with help and despite her aging teeth, Jackie has quite an excellent appetite for soft and tasty treats. She is fine being picked up, carried and appreciates the lap. She does not seem scared of other dogs and nears them without problem. Jackie came with the ailments related to her age. She is holding quite well at…….17 years of age…equivalent to 119 human years…Not bad, right ? Jackie is looking  for a new home, a new family who will offer the care and the love her former owners can not give her anymore.  Will you be the one ? Jackie is at the Manhattan Care Center.

Jackie is a 17 year old Chihuahua whose owner left her with family that was not able to care for her due to personal complications.
Jackie is calm around strangers when they come over and tends to just ignore them and go about her business. She was in the finder’s home who also had a 6 year old Chihuahua that Jackie was relaxed and playful with.
Jackie does not poop indoors but sometimes pees on herself and cannot always hold it. She does not destroy household items. She does not mind having her food, toys or treats touched. She is fine with being bathed and brushed. She barks when she hears people knock or ring the doorbell.
Jackie has a low level of activity at home and likes to follow her owner around the house but does not play with toys. She is fed dry food twice daily. She is house trained to use the bathroom outside but sometimes pees on herself. She does not pull on the leash when being walked.
Jackie kept her tail tucked during intake and was tense but allowed all handling for collaring, scanning and photograph.
