Medical Summary Estimated age: ~13yrs, spayed Microchip noted on Intake? 9855121003527306 Stray, reportedly finder tried shaving around anus and caused irritation QARH. Attention seeking, allows all handling. Excellent appetite EENT: Immature cataracts OU, significant amount of debris AU (dark brown firm mass like clumps of debris), minimal otitis, no nasal discharge noted Oral Exam: Severe dental tartar/gingivitis, gingival recession PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female MSI: Ambulatory x 3, non-weight bearing lameness RH, grade 4/4 MPL, matted fur restricting normal extension of limb, skin free of parasites, no masses noted, matted hair coat; BCS 1/9 CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Not performed, inflammed raw skin at perianal region Assessment: Geriatric, dental disease, emaciated, matted, RH MPL Plan: Pictures taken for cruelty group. Medical shave down, ears cleaned with debris removed, otibiotic applied to anal region. Gave rimadyl 6.25mg PO. Rec dental prophylaxis with placement Rewigh tomorrow for continued rimadyl dosing. Prognosis: Good prognosis with appropriate treatment and care SURGERY: Already spayed
