Medical Summary 01/14/16 12:30 INITIAL EXAM MICROCHIP SCAN NEGATIVE BAR, hydrated, active and energetic, very friendly on exam male, intact with two descended testes, smooth and symmetrical BCS 2/9 – underweight, diffuse muscle wasting, ribs and spine visable mild tartar build up on teeth, eyes and nose clean no discharge, clean ears AU no CIRDC signs heart and lungs WNL on auscultation dry skin, full haircoat peripheral lymph nodes all WNL dew claws on both hind legs abdomen soft and non-painful no masses palpated PLAN: gave rabies vaccine, DA2PP, CIV and bordetella gave 8 ml pyrantel PO applied activyl topically OK to start TID feeding

ACO reported dog has Diarrhea, watery stool seen in cage; Parvo test negative; As per on call vet Metronidazol given 750 mg bid po for 5 days;
Exam (1/16/16)
Rounds: Lethargy with glazed eyes and vomit in cage; A: CIRDC/V/D and underwt; P:Baytril 100 IM SID 10d; Cerenia x 3d; already on diarrhea meds
Group Behavior
Jack approaches the gate to greet the other dogs, standing tall with a wagging tail. He lived with a female dog and his owner reports he “went to the dog park regularly. He was relaxed, playful, tolerant, and played exuberantly with his sister and dogs of all sizes/sexes at the dog park.” However, because of his CIRDC he is not introduced. Based on his greeting and owner information, the behavior department recommends Jack have a pre-adoption interaction before being placed into a home with other dogs.
