I am a male tan shih tzu mix. The shelter thinks I am about 7 YEARS old.
I came in the shelter as a STRAY on 05/04/2016 from NY 11230, owner surrender reason stated was STRAY.

05/04/2016 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 13.2 LBS.
05/04/16 16:22 Re-exam for shave. Cardiac auscultation revealed grade 4/6 L and R systolic murmur. Grade 3/4 dental disease. Attempted to shave matts w/o sedation, Jabba would not allow. Attempted to bite clippers after a few mins. Sedated w/ 0.35ml Telazol IM. Mainted on mask oxygen. Clipped hair and matts. Cleaned ears. Applied eye lube and flushed eyes after clipping – unable to perform schirmer tear test today. Was coughing under sedation intermittently A: Heart murmur, dental disease, suspect KCS OU. P: Recommend NH plea for placement and veterinary follow-up. Jabba may require thoracic radiographs, and an echo prior to undergoing anesthesia for dental or neuter. Recommend min database base and cardiology consultation. Permanently waived from surgery at ACC due to heart murmur. Helper dog: A1072380 Jabba came in as stray, so his past behaviors in a home are unknown. At intake it was reported Jabba was nervous but did allow all handling. During SAFER Jabba came into the assessment room nervous and a bit fearful, he was friendly and social towards the assessor with a slow approach. He allowed all handling items to be conducted without issue. Jabba was relaxed during the tag interaction, and during the resources portion of the assessment Jabba was easy to handle when engaged with the item. Jabba has been showing fearful behavior and avoids interaction in the care center. We cannot be certain if behavior may be suppressed as a result of the shelter environment. Please note that behavior may change when Jabba is more comfortable and settled in a home environment. In addition, we recommend that introductions are done slowly, and Jabba be given the opportunity to interact at their own pace. The behavior department feels he can be suitable for an experience adopter. Look: 2. Dog’s eyes are averted. His body posture is fearful, trembles and his tail is low and not moving. He allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 2. Dog stands still and accepts the touch, his eyes are averted, his tail is between his legs, body a bit tense, mouth closed. Tag: 1. Follows at end of leash, body soft. Squeeze 1: 3. Dog closes mouth, becomes tense. Flank squeeze 1: 1. Dog does not respond at all. Flank squeeze 2:1. Dog does not respond at all. Toy 1: No interest. Dog-dog: 2. Dog approaches helper dog with tail at spine level, body not stiff, ears relaxed, and lip neutral.
