Re-Exam (2/02/16)
Hx: Pt brought in with wound on left hind leg from at-home grooming. Pt sedated for grooming and better exam of wound.
The wound is a triangular full-thickness skin avulsion (each leg of the triangle is 3 cm). The skin flap and the underlying tissue are pink; the skin flap is slightly thickened although the tissue appears healthy. The pt is extremely matted overall, with mats encircling all legs. A large amount of fur was found in both ear canals, mixed with yellow dry flaky debris. Pulled most of the fur out, but stopped when pt woke up from sedation. Cleaned the left leg wound with dilute chlorhexidine, applied SSD and bandaged with Tegaderm. Prognosis: Excellent.
P: Change bandage every other day or whenever it falls off. Clean ears, no ear tx at this time, monitor to determine whether a true otitis exists

sedated grooming and wound treatment. weight: 18 lbs; heart and lungs WNL on auscultation; gave 0.1 ml TTDex IM at 3:16 pm
Re-Exam (2/04/16)
S/O: vet check for re-check ears. mild light brown discharge AU. wound on left hind leg appears to be healing appropriately, dog seems comfortable and non-painful, nervous but allowed exam with treats and petting.
A: hx severe matting, wound on left hind leg, mild otitis externa (acute)
P: clavamox 125 mg PO BID x 10 days; rimadyl 12.5 mg PO BID x 3 days; sign up for ear cleaning twice weekly – rec follow up with private vet after placement
Behavior
Isaac came into our center after a member of the public found him as a stray. He was scared and wary of strangers upon intake, tense and nervous, tried to bite. It should be noted Isaac arrived at the center severely matted, but once groomed appears to be less fearful and less reactive. Isaac was social and interactive upon entering SAFER assessment room, body wiggly and in constant motion. He held gaze for three full seconds during Look item, body soft. He displayed high energy and movement during Sensitivity item, focused on getting closer to assessor, open mouth and fast wagging tail. Isaac initially followed at the end of leash with soft body and wagging tail, but became uncomfortable during each pass of Tag game, quickly spinning towards hand, did not escalate and approached assessor at end of game affectionately. He did not pull his paw back on both Squeeze attempts, tail wag. Isaac showed minimal interest in Toy. When off leash with other dogs Isaac is solicitous and polite. He approaches other dogs with loose body and wagging tail in attempts to engage them in play, but he walks away when they are not interested. The Behavior Team feels Isaac would do best with an Experienced adopter. Isaac will require regularly scheduled professional grooming appointments, approximately every 8-10 weeks, to avoid matting and discomfort.
Look: 1. Dog holds gaze with soft eyes, soft body. He allows head to be held loosely in Assessor’s cupped hands. Dog holds gaze for three full seconds.
Sensitivity: 2. Dog displays high energy and movement, but it is directed toward getting closer to the Assessor. His body is soft, open mouth, tail wag.
Tag: 3. Dog repeatedly spins toward the touch
Squeeze 1/ 2: 1. Dog does not respond at all for three seconds. His eyes are averted and his ears are relaxed, tail wag.
Toy: 1. Minimal interest in Toy, sniffs and mouths, then turns away.
Dog-dog: When off leash with other dogs Isaac is solicitous and polite. He approaches other dogs with loose body and wagging tail in attempts to engage them in play, but he walks away when they are not interested.
