INDIGO – 9782

**MUST HAVE PLACEMENT BY NOON OCT 16, has congenital heart failure**

8y;  Tan Female 3lbs.  Intake: Stray 10/14/17

Medical: 10/15 DVM Intake Exam; Estimated age: 8yrs; Microchip noted on Intake? History : Subjective: eating in cage earlier; Observed Behavior -allows all handling; Objective P =120 R =40 BCS 5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: oligodontia with focal severe dental tartar in L caudal dental arcade PLN: No enlargements noted; H/L: NSR, no arrhythmias, but grade 3/6 HM, possible increased lung sounds, increased respiratory effort at rest, CRT < 2, ; ABD: Non painful, no masses palpated, n opalpable fluid wave; U/G: FS, palpable spay scar, no tattoo seen; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, moderate pitting edema in LFL, R HL, R HL; CNS: Mentation appropriate – no signs of neurologic abnormalities; lethargic; Assessment; MI; HM: r/o valvular, myocardial with R congestive heart failure pitting edema limbs: r/o secondary to R sided CHF, hypoproteinemia, vasculitis, other oligodontia with moderate dental dz lethargy: r/o secondary to underlying cause; gave one dose Lasix 50mg/ml: 0.1ml IM (unable to access vein), consider repeat in AM, although ideal treatment is with O2 support PRN, spironolactone, hydrochlorothiazide, cardio consultation,- explained case to management-ideally would be transferred for intensive care ASAP, as ACC cannot treat here and without treatment will continue to worsen; Prognosis: poor- grave given clinical presentation
Plan: 2v full body rads: poor serosal detail in abdomen, enlarged cardiac silhouette with increased opacities in lung fields (not ideal positioning d/t p condition did not want to stress) cbc/chem in house- attempted, couldn’t locate veins to pull blood, didn’t want to stress at this time given p’s labored breathing, R CHF and inability to treat at ACC, rec transfer to hospital for intensive care vs. humane euthanasia
SURGERY: Permanent waiver due to cardiac disease
