DVM Intake Exam; Estimated age: 10 yo 6m; Microchip noted on Intake? neg; History : stray; Subjective: BAR. A bit nervous, placed muzzle just in case, but friendly during PE, no signs of aggression. Did not like rear end being touched. Observed Behavior – Objective ; T =; P = wnl; R = wnl; BCS 3.5/9; EENT: Eyes clear, ears clean, inner pinna hyperpigmented, no nasal or ocular discharge noted; Oral Exam: moderate calculus; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: soft Non painful, no masses palpated; U/G: MI, 2 testicles; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, white muzzle, small 1cm pendulous skin tag cranial to penis; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: normal externally; Assessment; geriatric; slightly underweight; intact male; skin tag; Prognosis: good; Plan: ctm until transfer/adoption; SURGERY: permanent waiver due to age
