Medical Summary 05/23/17 DVM Intake Exam Estimated age: 10 yrs Microchip noted on Intake? Yes #4355524156 History : Stray dog, noted to have mass on leg. Subjective: Observed Behavior – Calm, friendly, allows all handling. . Objective: BARH. mm=pink, moist, CRT<2 s. HR=124, RR=pant, BCS=4/9. ORAL: Mild to mod. calculus, dental staining. EENT: Nuclear sclerosis OU. No oculonasal discharge, no sneezing. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/organomegaly palpated. U/G: Female, spay scar found. MS: Ambulatory x 4, mild right front leg lameness. INTEG: Large, firm, non-mobile mass on right front let along entire lat humerus extending to caudolateral elbow. Not painful on palpation. 4 cm soft, SQ mass right lat. thorax. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Geriatric dog with large mass on right front leg. DDx: Fibrosarcoma, hemagiosarcoma, liposarcoma or invasive lipoma, osteosarcoma (although would ex[ect more discomfort), other. P: Scheduled for CBC/CHEM and radiographs of leg and thorax under sedation. Rx Rimadyl 75 mg. 1 Po q 24 hours x 7 days. Guarded prognosis pending results.
