DVM Intake Exam; Estimated age: 12 years; Microchip noted on Intake? no; History : stray; Subjective: BAR, hydrated; Observed Behavior – Very friendly allows exam/treatment with minimal restraint. Objective ; P =120; R =wnl; BCS: 5/9; EENT: NSOU, ears clean, no nasal or ocular discharge noted; Oral Exam: grade 4 periodontal disease, oro-nasal fistula where 104 was.; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: male intact; MSI: Ambulatory x 4, arthritic gait, skin free of parasites, no masses noted, healthy hair coat. Small lipoma sternum (2 cm); CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: oligodontia; periodontal disease; oro-nasal fistula; Prognosis: good; Plan: recommend periodontal therapy; soft food only; continue to monitor at ACC; SURGERY: permanent surgical waiver due to age
