Medical Summary Estimated age: 7 Microchip noted on Intake? no History : stray Subjective: BAR, hydrated Observed Behavior – quiet, friendly, easily examined and handled. Evidence of Trauma seen – yes Objective P = 120 R = 20 BCS: 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: worn, mildly stained dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact, both testes descended and symm. MSI: Ambulatory x 4, lame right rear and right forelimb. No obvious fractures or luxations. Foot injuries to both right front and right hind. swelling/ulceration/infection. small healing laceration pad of digit 5 right rear. Skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: injuries to right front paw and right rear paw, will need medical treatment, antibiotics/analgesics. consider bandage and or surgical closure of wound to right hind paw/pad. Plan: wound cleaning analgesics antibiotics Prognosis: good SURGERY: Okay for surgery , consider wound closure right rear while under for neuter.
