DVM Intake Exam; Estimated age: 16; Microchip noted on Intake? no; History : owner surrender -urinating more than usual; Subjective: BAR; Observed Behavior – Wagging tail and allows petting, very nervous when examined; Objective ;T = 101; P =; R =; BCS 6/9 ;EENT: Eyes NS clear, ears clean, no nasal or ocular discharge noted; Oral Exam: severe dental disease, loose teeth, recessed gingiva.  PLN: No enlargements noted;H/L: did not auscult an obv murmur (trembling) NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: soft, Non painful, no masses palpated,;U/G: spayed female;MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty hair coat;CNS: Mentation appropriate – no signs of neurologic abnormalities;Rectal: normal externally ;Assessment: Geriatric patient ;Severe dental disease; reported freq urination r/o UTI vs stones vs other; Prognosis: Fair; Plan: Rec senior bloodwork with urine sample ;Rec dental consultation, cleaning and xrays.

Housetrained: Yes Energy level/descriptors:  Honey is described her as friendly, affectionate, playful, and mellow with a medium level of activity.    Date of assessment: 26-Mar-2018

Look: 1. Dog’s eyes are averted, ears are back, tail is down, relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands.Sensitivity: 1. Dog leans into the Assessor, eyes soft or squinty, soft and loose body, open mouth.Tag: 1. Dog follows at the end of the leash, body soft.  Paw squeeze 1  1. Minimal interest in toy, dog may smell or lick, then turns away. Summary: Honey approached the assessor in the assessment room with a soft body. She was social during the assessment, allowed all handling, and displayed no concerning behaviors.Summary (1):  3/26: When introduced off leash to dogs Honey is tolerant of polite approach but mostly seeks to interact with human handlers.  Summary:  Honey allowed all handling.   Summary: Honey was tense but allowed handling.

Basic Information: Honey is an older female spayed medium size dog. She has been with the owner since a puppy. Owner is surrendering due to no time. Owner reported Honey urinating more frequently. The last time Honey saw a vet was back in November for annual checkup.Previously lived with: one adult How is this dog around strangers? Honey is friendly and outgoing. She allows to be pet. She will run up to strangers and wag her tail.  How is this dog around children? Honey has spent time with children and was relaxed, respectful, and playful with them. How is this dog around other dogs? Honey is afraid of large dogs but dogs her size and under she is playful and relaxed with them. How is this dog around cats? Honey has not been around cats so behavior is unknown. Resource guarding: There are no resource guarding concerns.  Bite history: there is no bite history with honey Housetrained: Yes. Energy level/descriptors: medium. Other Notes: Honey isn’t bothered with bath time or brushing of coat. it is unknown how she will react with trimming of nails. She is friendly with unfamiliar approaches to the owner and home. Medical Notes:  Owner reported Honey urinating more frequently for the last month. For a New Family to Know:  Owner described her as friendly, affectionate, playful, and mellow. She doesn’t care for toys anymore. She eats both wet and dry food blue buffalo. She like puperoni treats. She is well behaved being left alone in the house. She knows sit, come, stay, paw, and give kiss. She pulls lightly on leash. When off leash she stays close to owner’s side.
Behavior Notes:  Upon intake, Honey had a neutral body language. She allowed to be collared, scanned, and took treats gently. She allowed to be pet all over.
