DVM Intake Exam; Estimated age: 8 months; History : stray, found roaming the halls of a building with 2 other dogs; Subjective: Observed Behavior – friendly; Evidence of Trauma seen – scabs at face; Objective; BCS 4/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: clean adult teeth; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: FI; MSI: Ambulatory x 4, skin free of parasites, no masses noted, multiple small scabs on muzzle, superficial puncture wound at forehead; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment; multiple scabs and superficial puncture wound- R/O bite wounds vs other trauma; Prognosis: good; Plan: did not give Rabies vaccine until DOH status is determined; SURGERY: Okay for surgery
