Medical Behavior Evaluation BLUE
Medical Summary Estimated age: 1.5 yo F Pitt Microchip noted on Intake? scan neg History : o surrender – abandoned by a friend. No known health issues. Subjective: BARH Observed Behavior – Inquisitive. Allows all handling. T = P = 140 R = eup BCS= 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Moderate underbite – brachynathism. Clean teeth. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female, no spay scar seen. No mammary masses. No vulvar d/c. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat except for several (7-10) approx 1 cm papules in lower abdomen and inguinal area. No signs of dermatitis along mid- line where spay incision would be but a couple about 3 cm lateral to midline. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Assessment: 1.5 yo F Pitt Mild dermatitis Plan: Cephalexin 500 mg 1 po bid x 10 days. Prognosis: Good. SURGERY: Temporary waiver due to dermatitis. Re-check in 1 week.

PROFILE :
Basic Information:
Hersheys is a brown colored, female large dog. He was surrendered for personal reasons. She last went to the vet around a year ago and has no known health problems or injuries.
Socialization:
Hersheys is shy for a few minutes then becomes friendly and outgoing. She spent time around children ages 10, 11 and 12 and was relaxed, playful and respectful around them. She was gentle when playing with children and adults. She is playful with dogs of all sizes. It’s unknown how she is with cats.
Behavior:
Hersheys is generally well behaved. She isn’t bothered with loud noises or fireworks or if you touch his food, bones or any toys. She is friendly if given a bath. She is friendly when a stranger approaches the home or family member.
For a New Family to Know:
Hersheys is mellow, confident and affectionate to his owner with a medium activity level. She loves to just hang out and lay next to his owner. She loves dog bones and was fed dry food (Beneful). She goes potty on gravel/dirt during walks and understands the commands sit, stay and get down. She pulls hard on the leash but stays by your side when off leash.
Behavior during Intake
Hersheys maintained a soft posture but sort of withdrew towards owner when counselor approached. She however relaxed some when given a bone treat and allowed petting and all handling.
GROUP BEHAVIOR :
9/16: When introduced off leash to other dogs, Hersheys is uncomfortable with play solicitation.
