DVM Intake Exam; Estimated age: 10yrs (reported); Microchip noted on Intake? N; History : owner surrender (client’s mother passed away, she cannot care for the dog); Subjective: BAR; Observed Behavior – complaint for exam, tolerant for vaccines, **tries to bite if paws touched**; Objective ; T = DNP; P = WNL; R = WNL; BCS 6/9; EENT: Pigmentation of corneas OU but visual, mild mucoid discharge OU, ears clean, no nasal or ocular discharge noted; Oral Exam: Moderate dental dz w/ diseased/mobile lower left premolar; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Intact male- both testes present in scrotum; R testicle larger than L; MSI: Ambulatory x 4, skin free of parasites, no masses noted, matted hair coat especially base of ears (L>R) and inguinal region; CNS: Mentation appropriate – no signs of neurologic abnormalities; intact palpebral/menace OU; Rectal: DNP; Assessment: geriatric, dental dz w/ mobile tooth that needs extraction, asymmetrical testicles, adult intact male dog w/ possible food allergies (reported); Prognosis: fair to good; Plan: rec dental cleaning w/ extraction(s), ok for placement and neuter; SURGERY:  Permanent waiver due to age (geriatric).
