Medical Behavior Evaluation BLUE
Medical Summary Scan neg. Reported to have been seen on the streets yesterday and was seen getting hit by a car. QAR, Nervous, tried to snap. BCS 4/9. EENT: Eyes clear, ears clean, no nasal discharge noted. Oral Exam: Mild dental tartar. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic. ABD: Not palpated. U/G: Male. MSI: Painful in hind end, no palpable fractures, reluctant to stand/walk, unable to assess gait at this time, skin free of parasites, no masses noted, slightly dirty hair coat. CNS: mentation appropriate – no signs of neurologic abnormalities. Assessment: reported hit by car with pain in hind end. Plan: Administered hydromorphone 0.2ml IM. No interest in food at this time. Will continue hydromorphone, reassess tomorrow, +/- radiographs. Good prognosis at this time
RE-EXAM
03/07/17 : Re-exam, reported HBC, radiographs. S/O: QAR. Nervous, allows handling. BCS 4/9. EENT: Severe dental tarar buildup, mild gingivitis, very early nuclear sclerosis, no ocular or nasal discharge, ears wnl. HL: No murmurs/arrhythmias, lungs clear, HR 174, normal RR/RE. ABD: Normal abdominal palpation, large soft bladder. INTEG: Slightly dirty hair coat, minor abrasions at distal hind limbs, mild bruising at inguinal region. MS: Very reluctant to stand and walk, favors RH limb. UG: Male intact. Sedated with telazol 0.1ml IM for full exam and radiographs. Radiographs: lateral and VD pelvic — complete fractures of the right ischiatic spine, pubic bone and pelvic symphosis.
A: Reported hit by car yesterday, Right sided pelvic fractures. Est age 7-10yrs
P: Continue with current pain management. Rec surgical consult. Fair/good prognosis with appropriate treatment and care. Rec dental

SAFER ASSESSMENT: 03/08/17
Due to his current medical condition/health status, Herman is not an appropriate candidate for a SAFER assessment at this time. The behavior department believes Herman would benefit best from placement with a New Hope Rescue who can meet his needs medically and then assess behavior once he is in a stable environment. Force-free, reward based training is advised when introducing/exposing Herman to new and unfamiliar situations.
