Medical Summary DVM Intake Exam Female intact Estimated age: 6 years Microchip noted on Intake? negative History : Stray dog. Bandaged right hind limb. Subjective: Female intact Shih Tzu, matted hair and a sof t bandage on the right hind limb. Observed Behavior – Trying to bite if the right hind limb was touched. Handler should approach slowly. Tolerated leash on hher neck. Evidence of Cruelty seen – Bandage on the right hind limb, matted. Evidence of Trauma seen – Right hind limb fracture. Objective T = 102.3 F P = 140 bpm BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Noot examined PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact female MSI: Ambulatory x 3, right hind limb non weight bearing lameness, no masses noted, matted hair coat specially every extremity. Possible tial fracture. Extremely inflammed and infected right hind limb starting from the middle on the tibia down to the digits. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment Tentative diagnosis- Infected tibial fracture Plan Vaccinated against DAPPv, Bordetella and Rabies. Sedation- 0.1 ml telazol IM, Mantained with Isoflurane 2.5 Body was shaved, bandage removed and wound cleaned. 0.25 ml hydromorphone IM once 0.5 ml convenia sq 0.3 ml enrofloraxin IM ( 30 mg) once 0.4 ml carprofen SQ once ( 5mg/ml) Right hind limb amputation Fluid therapy is recommended. Prognosis: fair Hospitalization- Blue Pearl Temporary waiver due to fracture
