I came in the shelter as a STRAY on 05/21/2017 from NY 11208, owner surrender reason stated was STRAY.

On rimadyl, clavamox and baytril Open prognosis depending on underlying disease process — poor for MGT, fair for severe mastitis; both will require surgical intervention

A staff member writes:Oh Heaven! Heaven is just what she is. She is one of the most sweetest, prettiest, kindest, most nurturing dogs I have ever come across. She has such a big heart and she displays it everyday through every course of interaction. It was love at first sight for me. Since she has been at the shelter, I’ve dedicated my time to her. Every moment I’ve spent with her, I love her more and more. She loves to be hugged, it gives her a sense of reassurance and she also loves rubs, she has this one spot in particular that will have her smiling, and that smile of hers is VERY INFECTIOUS!!! She loves long walks and exploring her surroundings. When we go for walks, I make sure it is a time to remember for her. I look forward to seeing this love muffin when I come into work everyday, BUT I look forward to seeing Heaven going into her forever home even more, she needs someone that will appreciate her and everything she comes with because I know for a fact she will make any new family(s) life worth living. I know first hand, she lights up everyone’s world with her gracious presence. KNOWN HISTORY: None 5/21/17 Unaltered Female, Stray Other notes: Upon intake Heaven was calm and allowed all handling. SAFER ASSESSMENT: 5/24/17 Look:1. Dog’s eyes are averted. Her ears are back, her tail is down, and she has a relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog leans into the Assessor, eyes soft, soft and loose body, open mouth. Tag:1. Follows at end of leash, body soft. Squeeze 1:1. Dog does not respond at all for three seconds. Her eyes are averted and her ears are relaxed. Squeeze 2:1. Dog gently pulls back her paw. Toy:1. No interest. Summary: Heaven was very friendly and affectionate towards the assessor. PLAYGROUP: Heaven displays soft body language when greeting a novel male dog. She is social, checking in on him often as she explores the pen. MEDICAL BEHAVIOR: 5/21/17 During her medical examination, Heaven was friendly and allowed all handling. ENERGY LEVEL: We have no history on Heaven so we cannot be certain of her behavior in a home environment. However, she is a young, enthusiastic, social dog who will need daily mental and physical activity to keep her engaged and exercised. We recommend long-lasting chews, food puzzles, and hide-and-seek games, in additional to physical exercise, to positively direct her energy and enthusiasm. RECOMMENDATIONS: Average (suitable for an adopter with an average amount of dog experience)

Not trying to move. Evidence of Cruelty seen – None Evidence of Trauma seen – Left mammary gland laceration Objective T = 102.3 F P = 100 bpm BCS 5/9 EENT: Eyes clear, ears- brown discharge, no nasal discharge noted Oral Exam: Not examined PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, Mas. Deep leaceration on the left mammary gland. 5 inches deep. Can palpate the torax. Producing milk from the mammary glands. U/G: intact female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NAF Assessment mammary gland laceration ( wound) Plan Flushed with chlorhexidine solution Will take radiographs tomorrow. 375 mg clavamox BID PO for 14 days. Prognosis: guarded SURGERY: Okay for surgery
