08/29/2016 PET PROFILE MEMO
08/29/16 15:45 Basic Info: Hazel A1087707 is a 10yr old female white & tan AMPBT mix who was purchased from someone selling puppies 10yrs ago and is being surrendered due to her biting a neighbor and them telling the owner to “get rid of her”. She was last seen by a veterinarian as a puppy and currently has no known illness/injuries. Socialization: Around strangers Hazel is friendly/outgoing. She hasn’t been around young children but did live with a younger male Pitbull and a Pomeranian (didn’t interact with him because the owner was afraid she wouldn’t like him) and was gentle, exuberant, relaxed & playful with the Pitbull and the adults in the home. Hazel bit a neighbor who walked past her as she was tied up outside and barking at the direction of the neighbor (they couldn’t tell me if it was her just barking or if it was an aggressive/excited/anxious bark). They walked by her, she bit them once on the ankle and let go. Behavior: Hazel isn’t bothered by the following: having her food/food bowl, toys/treats touched/moved, being moved off of furniture or being brushed. She hasn’t been disturbed while sleeping/resting or had her nails trimmed. For bath time Hazel will jump into the tub (she loves water). If someone approaches the home/family member Hazel will bark. For A New Family to Know: Hazel is described as friendly, affectionate, playful, confident, mellow & excitable (during play) with a medium activity level. She enjoys sleeping/resting & playing. She enjoys squeaky toys, games of fetch, chase & tug sometimes. Hazel is an indoor/outdoor dog who sleeps with one of the adults & is walked 1x a day where she pulls on her leash at times. She eats dry Pedigree 2-3x a day, is house broken & is crate trained (can be in one for 8-9hrs). When left home alone Hazel is well behaved and her known commands are “sit”, “stop”, “stay”, “give paw (both paws) and “come”. She is very smart. Intake Behavior: Hazel had a loose wiggly body and allowed all handling. She allowed to be scanned (-), collared and taken pictures of. Hazel was then walked to her kennel where she was asked to go in and she did.

09/07/2016 BEHAVIOR EVALUATION – EXPERIENCE
Exam Type BEHAVIOR
Hazel pulls a bit on the leash. She was not sociable, and was more interested in the surroundings than the handler during the assessment. Hazel was shy during handling with a fearful body language. She was distracted, focused on objects in the room rather than the handler during the tag test. Hazel was not interested in toys. Hazel rushed in quickly during the dog-dog test, but showed no aggression. In shelter, Hazel is at the front of her kennel when approached and is easily leashed. She pulls hard to get outside and relieves herself immediately. She is taken off leash in a pen and spends a majority of her time exploring. She will occasionally approach the handler when called then sit at their feet, accepting petting. On one ocassion, when a female dog passes by, Hazel begins to run the fence and bark; she is distracted by handler interruption and recovers easily. The behavior department feels that she may do best with an experienced adopter. Hazel is somewhat fearful and needs time to warm up to the handler. We also recommend a home without young children; older children can be considered pending interaction. Look: 2. Dog’s eyes are averted. Her body posture is fearful; her tail is low and wagging . She allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 2. Dog stands still and accepts the touch, her eyes are averted, shy body language , mouth closed, lip long, ears likely back, may lip lick. Tag: 2. Dog is not fearful, but is unresponsive to the Assessor, and approaches the Assessor at the end of the game (may need coaxing to approach.) she is focused on stimuli other than the Assessor. Squeeze 1/2: 1. Dog gently pulls back her paw. Toy 1. No interest. Dog – dog 2. Dog approaches the helper dog by rushing in with her tail at spine level, body not stiff, ears relaxed, and lip long Helper: A1088209
