07/23/2016 PET PROFILE MEMO
07/23/16 12:50 BASIC INFO Hazel A1082361 is an approximately 4 year old American Pitbull Terrier Mix. She was a gift to the owner from friends, 4 years ago. The main reason for surrender is that the owner cannot have pets in the building where, he resides. Hazel does not have any apparent health concerns. SOCIALIZATION Hazel is friendly with strangers. When people come to visit the home, she greets them. Hazel has never lived with another dog. Hazel plays gentle with adults. Hazel has not lived with another animal. Hazel has never bitten anyone. BEHAVIOR The only behavior issue that Hazel has is that, she has separation anxiety when her owner leaves her to go to work. Hazel allows you to take away her food bowl, water bowl, or toy away. She allows you to bathe her but does not like to get her head or face wet. If an aunfamiliar approaches the doorway, she barks. FOR A NEW FAMILY TO KNOW Hazel is best described as friendly, affectionate and playful. She has a high activity level. When home, she follows you around. She enjoys playing with stuffed toys and balls, ropes and teddy bears. Hazel has been kept mostly indoors. She sleeps on her doggy bed. She was fed organic food, Rachel Ray brand. She enjoys Milk Bone doggy biscuits as a treat. Hazel is very house trained, and wee wee pad trained. When left alone in the home, she barks. She listens to the commands sit, stay and down. For exercise she is walked on the leash. BEHAVIOR DURING INTAKE Hazel was panting. She did allow handling and photos.

07/28/2016 BEHAVIOR EVALUATION – NH ONLY
Exam Type BEHAVIOR
Hazel was surrendered by her owner due to landlord issues. Hazel is reported to be friendly with greeting strangers and will play gentle with adults she familiarizes herself with. Her owner states that Hazel has never lived with another animal, behavior around other animals is unknown. Her previous owner reports that the one issue she does struggle with is separation anxiety, other than that she has shown no concern. He best describes Hazel as friendly, affectionate and playful with a high level of energy. Hazel is also reported to be very housebroken. Due to safety and handling concerns, Hazel is not an appropriate candidate for SAFER at this time. Hazel has displayed a low threshold for aggression in shelter and is easily startled, frightened and intimidated by any form of human engagement and interaction. Direct eye contact seems to present an immediate threat to Hazel causing her to react by lunging, growling, and even attempting to bite. The behavior department recommends Hazel be placed with a New Hope Partner who can follow up with this behavior as well as present a unique plan to help Hazel thrive and succeed. Because her owner profile is in stark contrast to what we have seen in the shelter, we cannont with much confidence predict how she will be in a new home environment.
