I came in the shelter as a OWNER SUR on 06/02/2016 from NY 11221, owner surrender reason stated was MOVE2PRIVA.

Dog was brought to medical after having a mild seizure during playgroup, dog is currently BAR, transferred to MACC for overnight observation

06/02/16 19:30 Basic Info Hazel is a 10 year old dog that was surrendered because she can no longer keep the animal in her home any longer. Hazel has never had any injuries however hazel does suffer from seizures. Client stated that when hazel was taken to the vet they didn’t say what causes the seizures. Socialization Hazel isn’t very friendly with dogs, cats or people. Hazel is reactive to both animals and people he is not familiar with. Hazel had never had an aggressive interactive with anyone or animal. When hazel is around people he does not know he will growl and bare back. When Hazel is around children he will growl. Behavior Hazel has no pervious bite history with anyone or animal. Hazel is very protective of the family members that live inside the home. Hazel has been to the groomer for baths and nail trimmings. Hazel is food reactive. Hazel does not allow his food bowl to be touched or taken away. When attempted he will growl and then bare back. For a new family to know Client suggested that hazel be placed in an experienced home. Hazel eats dry foods. Hazel food is left out the entire day. Hazel does pull on the leash when being walked. Upon intake : Hazel barked at the counselors. Hazel was collared, photograph and caged by the owners.

06/04/2016 BEHAVIOR EVALUATION – EXPNOCHILD

Hazel previously lived with adults and has been around children. The owner stated Hazel isn’t very friendly with dogs and cats, and when around people he isn’t familiar with he will growl and keep back. It was reported by the owner Hazel When Hazel is around children he will growl. The owner mentioned Hazel does not allow his food bowl to be touched or taken away he will growl and bare back. At intake it was reported Hazel barked at the counselors, he allowed handling with assistance from the owner. During SAFER Hazel came into the assessment room anxious and panting, he was social towards the assessor but consistently whines. He allowed all handling items to be conducted without issue. Hazel follows but consistently whines during the tag interaction, and during the resources portion of the assessment Hazel was easy to handle when engaged with the item. Due to some concerns stated from the previous owner, the behavior department feels he can be suitable for an experience adopter with no children and no other animals in the house hold and we recommend introductions are done slowly. Look: 2. Dog’s eyes are averted. His body posture is fearful, whines, trembles with his tail is low and not moving. He allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 2. Dog stands still and accepts the touch, his eyes are averted, and his tail is between his legs, body a bit tense and whines. Tag: 1. Follows at end of leash, body soft but whines. Squeeze 1: 2. Dog quickly pulls back his paw. Squeeze 2:1. Dog gently pulls back his paw. Toy 1: No interest.

According to Hazel’s previous owner, Hazel is reactive but not aggressive towards other dogs. When off leash with dogs at the Care Center, Hazel greets and wanders in the pen. He becomes sexually motivated, mounting female dogs.
