Vet Notes: 2:30 PM: DVM Intake Exam – Estimated age: 8-9yrs Microchip noted on Intake? yes Microchip Number (If Applicable): History: O/S due to health issues Subjective: Dog is BARH Observed Behavior – easy to handle for exam and treatments; did not love ear cleaning (kept trying to pull towards door), but allowed it without any warning signs. Objective: T = NA P = wnl R = wnl. BCS 5/9 EENT: Eyes clear, ears moderate-severe erythema with lichenification and scabbing; mild amount light brown waxy debris, no nasal or ocular. discharge noted. Oral Exam: Grade 2-3/4 dental dz; 208 appears to be missing and or severely fx. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. U/G: perineal erythema and lichenification. MSI: Ambulatory x 4, skin free of parasites, no masses noted, reddish-brown staining of all four paws. CNS: Mentation appropriate – no signs of neurologic abnormalities. Rectal: not performed Assessment: Otitis externa, AU, chronic. Staining of paws and erythema and lichenification of. perineal area also consistent with chronic yeast dermatitis DDx: Food allergy, atopy, open Prognosis: Good with appropriate treatment Plan: Cleaned AU and applied Claro. Recommend derm consult on placement, which may include (but is not limited to) cytology, food trial, oral and/or topical anti-fungals, etc. SURGERY: Already spayed
