10/31/16 Geriatric exam O: BARH. Excited, social. mm=pink, moist, CRT<2 s. ORAL: Mod. calculus, gingivitis. EENT: Nuclear sclerosis OU. Slight serous discharge OU, no chemosis or scleral injection. H/L: Lungs clear, no murmurs. ABD/UG: Intact male. Mildly pot-bellied appearance. Palpation WNL. MS: Amb x 4. INTEG: Raised, 1 cm pink growth left dorsal muzzle. A: Dental disease. Small skin growth – r/o sebaceous cyst, adenoma. P: Good prognosis.

10/29/16 17:16 Basic Information: Hayang is a male maltese who was surrendered due to his previous owner moving to a place where animals are not allowed and not being able to find him a new home. Hayang only understands Korean. Socialization: Around strangers Hayang is shy for a few minutes and plays gently with adults. Hayang has never been around children or cats so it is unknown how he will react to being around them. Hayang has spent time with dogs of various sixes and is relaxed and playful around them. Hayang has never bitten another person or animal. Behavior: Hayang has no behavior issues and is not bothered by loud sounds. Hayang is friendly when someone touches his food or bowl while he is eating, takes a treat/toy away, pushes/pulls him off of the furniture, holds/restrains him, duisturbs him whiel he is resting/sleeping, gives him a bath, brushes him or trims his nails. If an unfamiliar person approaches Hayang’s family or home Hayang will bark. For a New Family to Know: Hayang is described as firendly, affectionate, playful, confident and excitable with a high activity level. When at home Hayang likes to stay in the same room as hi sowner and likes to play with balls, stuffed toys and squeaky toys. Hayang was kept indoors only. Hayang was fed dry food and wet food twice a day. Hayang is house trained and rarely has any accidents Hayang will usually go to the bathroom on wee wee pads. Hayang has never been crate trained and is well behaved when left alone. Hayang likes slow walks and brisk walks . Hayang pulls lightly while on a leash and will run away if off leash. Intake Behavior: Hayang was very nervous around staff and avoided being touched by staff.

Other notes: Nervous and avoided being touched at intake. SAFER ASSESSMENT: 10/31/16 Look: 1. Dog holds gaze with soft eyes, soft body. He allows head to be held loosely in Assessor’s cupped hands. Dog holds gaze for three full seconds. Sensitivity: 1. Dog stands still and accepts the touch, his eyes are averted, and his tail is in neutral position with relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item. Tag: 1. Follows at end of leash, body low and a bit fearful. Squeeze 1: 5. Dog growls or tries to bite Toy: 1.
