Medical Summary DVM Intake Exam Estimated age: 7yrs Microchip noted on Intake? positive History : o surrender prev o’s children had allergies to dog, reported to have KCS no meds provided Subjective: Observed Behavior – friendly, allowed all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = P = 120 R = sniffing but eupneic BCS 5/9 EENT: OS- moderate purulent d/c from eye, ears clean, no nasal discharge noted Oral Exam: moderate tartar, halitosis, malocclusion, underbite PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI both testicles descended, normal consistency MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment MI KSC with infection OS Plan schirmer tear test 15mm/min OD, and 7mm/min OS ok for adoptions and neuter TAB ointment OS BID x7 optimmune ointment QIS OS BID x14days recheck STT in 7 days once eye infection resolved long term rec dental Prognosis: fair for KCS, may progress and warrant additional meds/tx’s SURGERY: Okay for surgery
