Hx: Episodes of trembling and head bobbing, initially thought to be pain and then determined to be more likely to be seizures. Most recent observed seizure was 6/5/17 in the evening, lasted 30-60 seconds and then post-ictal period was 5-10 minutes. This morning, pt is running around, barking, allows all handling. Recommend starting antiepileptic when available. 1088

05/18/17 14:32 Socialization Around strangers, he is friendly and outgoing and allows everyone to pet him from head to tail. He has been around two toddler children and is relaxed and respectful with them. He doesnt pay them any mind so play behavior is unknown. He plays gentle and exuberant with adults. He has not been around other dogs so behavior is unknown. He has not been around cats either.there is no bite history of a person or animal. behavior Owner stated that he does get anxious when left alone. He isnt bothered with touching of food bowl, taking a bone or toy out his mouth, holding or restraining, bath time, brushing of coat, and unfamiliar approaches. It is unknown with trimming of nails. for the new family to know Owner described him as friendly, affectionate, playful, and mellow with a medium activity level. When owner is home he follows you around. He doesnt play much was kept mostly indoors and sleeps in a dog bed. he likes any type of treats. he is mostly house trained and rarely has accidents. when left alone in the home he will bark and whine. he is crate trained and does very well in it. he knows sit, come, and down. he doesnt pull on leash and owner has not had him off leash

06/04/2017 DVM INTAKE PHYSICAL EXAM
Medical rating was 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, behavior rating was NONE
DVM Intake Exam Estimated age: 8-12 years based on dentition, skin and apparent medical conditions. Microchip noted on Intake? Scanned positive Hx: Diagnosed with CIRDC on intake 4/14/17 due to nasal discharge and coughing. Pt was treated with doxycycline – still coughing, treated with azithromycin and cough resolved. After being adopted later in April 2017, pt had some strange episodes of trembling and head bobbing, appearing disoriented, inability to walk – seen by 1382 and diagnosed as seizure vs. back pain from possible IVDD. Treated with Cerenia, dexamethasone SP. Episodes continued – they last 15 min or so, and pt is completely normal and well-appearing after the episodes. Pt also spends time licking scrotum and paws. 1. Trembling episodes – likely pain and not seizure due to duration of episodes and lack of post-ictal period. R/O IVDD, spinal arthritis, other cause of nerve impingement. Recommended an MRI to evaluate pt’s spine and spinal nerves. 2. Dental disease – significant. Recommend dental cleaning and rads, likely extractions. This is not top priority right now but it should happen in the next few months, for pt’s comfort. Subjective: Barking and jumping around constantly. Doesn’t sit still. Objective BCS 4/9, MMs pink and moist EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Severe tartar and gingival recession H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male neutered, incision healing with no swelling or discharge MSI: Lumbar kyphosis, ambulatory x 4, jumping around. Thin, fluffy fur coat. Skin free of parasites. No masses noted. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 1. Reported episodes of pain or neurological signs – R/O IVDD, spinal arthritis, other cause of nerve impingement 2. Severe dental disease Plan: 1. Recommend spinal MRI 2.
