DVM Intake Exam; Estimated age:10 years; Microchip noted on Intake?n; History :stray; Subjective: matted; Observed Behavior -allows full exam, calm; Objective ; T =; P =60; R =pant; BCS 4/9; EENT: ou sclerosis; Oral Exam:severe dental disease; PLN: No enlargements noted; H/L: heart murmur 3/6; ABD: Non painful, no masses palpated; U/G:2 testes; MSI: severe matting, legs do not appear to be strangulated; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: heart murmur, dental disease, matted; Prognosis:good; Plan:extreme caution with sedation. recc echo cardiogram first, then grooming, neuter, dental; SURGERY: Permanent waiver due to age, heart murmur

Date of assessment:  10-Jul-2018

Look:  1. Dog’s eyes are averted, with tail wagging and ears back. Allows head to be held loosely in Assessor’s cupped hands.
Sensitivity:  1. Dog leans into the Assessor, eyes soft or squinty, soft and loose body, open mouth.
Tag:1. Dog follows at the end of the leash, body soft.
Paw squeeze 1:  1. Dog gently pulls back his/her paw.
Paw squeeze 2:  2. Dog quickly pull back.
Toy:  1. Minimal interest in toy, dog may smell or lick, then turns away.

Summary:  Google approached the assessor with a soft body. He was social during the assessment, allowed all handling, and displayed no concerning behaviors.

Summary (1):  7/10: When introduce off leash to dogs, Google keeps mostly to himself.

Summary:  Calm, allows handling

ENERGY LEVEL:  We have no history on Google so we cannot be certain of his behavior in a home environment. In the care center, he displays a medium level of activity.

BEHAVIOR DETERMINATION:  AVERAGE (suitable for an adopter with an average amount of dog experience)
