Medical Summary DVM Intake Exam 04/21/17 Estimated age: Owner reports 8yrs, appears older, ~10-12yrs, neutered History : Owner surrender, owner reports urinating brown urine in house Subjective: Observed Behavior – QARH. Slightly timid, used soft muzzle for vaccinations, allows all handling for exam Objective P = wnl R = wnl BCS 4/9 EENT: Immature cataracts OU, ears clean, no nasal discharge noted Oral Exam: Moderate dental tartar, and calculi build up, pink moist mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, Unkempt hair coat, appears to have been recently shaved along caudal dorsum CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric, cataracts, reports of brown urine Plan: Rec CBC/Chem/T4/UA scheduled for tomorrow. Continue to monitor Prognosis: Open prognosis until diagnostics are done. Good short term prognosis
